To arrange an out of warranty repair on one of the Fazzini suction pumps listed below, please complete the RMA and decontamination form from the links below.

The completed form should be sent to the repairs inbox -rsch.hpleme@nhs.net prior to dispatch. A printed copy of the form should also be included with the device, along with a decontamination form.

• Fazzini model F31 portable suction unit (10601)
• Fazzini model F31 portable suction unit with trolley
• Fazzini model F60 portable suction unit (10901)
• Fazzini model F60B portable suction unit (10801)

Please note: For In warranty repairs, please contact your local GASARC Area Sales Manager, who will provide the necessary arrangements for the device to be returned to Fazzini for work to be completed and returned back to your Trust.
